MEDAL FORMS: Veterans and military personnel wishing to receive a medal must complete the following Governor’s Veterans Service Award form and bring with them documentation or identification to validate their veteran status.

Types of documentation include DD214, retirement ID card, or VA ID Card. Please refer to the GVSA form for a comprehensive list. Click here to access the form.

Eligible personnel are strongly encouraged to complete the Governor’s Veterans Service Award form ahead of time and bring a copy with them to help expedite the registration process. Forms will also be available on site.

OUTREACH: Representatives will be on hand with resources and information to help you connect with your earned benefits, services and employment opportunities.

ATTIRE: Recommended attire is business casual. Military uniforms are not required unless directed by military chain of command.
